Well, maybe not completely unforeseen, because I'm foreseeing them right now. But I bet that CCP haven't considered the following:
Once T3 production gets underway, the petitions system collapses under the strain of people complaining about not only their ship losses but also their SP losses. Because they refuse to concede that their idea of SP loss was anything but wonderful, CCP eventually introduce a new and faintly ludicrous mechanic that allows SP to be insured, replaced, or just plain bought for ISK.
In April the forums, rookie channel and the petitions system all collapse under the strain of noobs wondering why their skills aren't training as quickly as they were. A mass exodus of new players results. |

That will be the crux of CCP's problem though. Because of the risk of SP loss, the overwhelming number of T3 ships will be used in high-sec, and CCP will inevitably look to make their use more common after the whines meet a certain level. |
